Library membership registration form

To register as a library member, you must agree to the Library Terms of Use. If you are under 18, your parent or guardian must also provide their consent and agree that they will be responsible for any fees that may be incurred.

When you register, we need to collect some personal information from you, which will be used to provide you with library services and for related administrative purposes. Monash University values the privacy of every individual's personal information and is committed to the protection of that information from unauthorised use and disclosure except where permitted by law.
